August 17, 2006, Release #06-237: 5-Gallon Zep-Brand Cleaner, Floor Stripper Sold at Home Depot Recalled, Containers Crack and Leak Corrosive Liquids-2006

WASHINGTON, D.C. - The U.S. Consumer Product Safety Commission, in cooperation with the firm named below, today announced a voluntary recall of the following consumer product. Consumers should stop using recalled products immediately unless otherwise instructed.

Name of Product: 5-gallon sizes of "Zep Industrial Purple Cleaner & Degreaser," and "Zep Heavy Duty Floor Stripper"

Units: About 6,800

Distributor: Acuity Specialty Products Group Inc., a subsidiary of Acuity Brands Inc., of Atlanta, Ga.

Hazard: 5-gallon plastic pails containing the cleaning products can unexpectedly crack and leak from the base, posing a risk to consumers due to the corrosive nature of these products.

Incidents/Injuries: Acuity Specialty Products Group Inc. has received three reports of leaking pails. No injuries or property damage have been reported.

Description: "Zep Industrial Purple Cleaner & Degreaser" is a clear, purple liquid. "Zep Heavy Duty Floor Stripper" is a clear, colorless, water-thin liquid. Both products were sold in 5-gallon, white pails with large blue labels and blue lids. Each pail carries a batch code written on the side of the pail. Only the following batch codes are included in the recall:

Industrial Purple Cleaner & Degreaser (HD0856), Batch Codes: 1607501, 1607601, 1608001, 1608101, 1608201, 1608601, 1608701, 1609001, 1609101, 1609401, 1609601, 1610101, and 1610301.

Heavy Duty Floor Stripper (HD1071), Batch Codes: 1607401, 1607601, 1608101, 1608201, 1608301, 1608801, 1609001, 1609701, 1610001, and 1610201.

Sold at: The Home Depot nationwide from March 2006 through April 2006 for between $32 and $35.

Manufactured In: United States

Remedy: Consumers should stop using the recalled products and contact Acuity Specialty Products Group Inc. for information on how to receive a free replacement pail. Consumers with a leaky pail should place it inside another 5-gallon pail or large plastic bag, and then pour the remainder of the contents into a new container and label the product. Use gloves and eye protection when handling. Clean any spilled material by absorbing onto a solid absorbent and sweep up and discard it into a solid waste container. Rinse the affected area with clean water until no residue remains.

August 31, 2005, Release #05-253: Tile Perfect Inc. Announce Recall of Stand’n Seal Grout Sealer Due to Respiratory Problems-2005

WASHINGTON, D.C. - The U.S. Consumer Product Safety Commission, in cooperation with the firm named below, today announced a voluntary recall of the following consumer product. Consumers should stop using recalled products immediately unless otherwise instructed.

Name of Product: Stand’n Seal “Spray-On” Grout Sealer sold in 2005

Units: About 300,000 cans

Distributor: Tile Perfect, a division of Roanoke Companies Group Inc., of Aurora, Ill.

Hazard: The product’s odor is not chemically pungent enough to force consumers to minimize their exposure to the fumes. Consumers overexposed to these fumes can experience respiratory-related illness.

Incidents/Injuries: There have been 88 reports from consumers who have had adverse reactions after using the aerosol product, including 28 confirmed reports of overexposure resulting in respiratory symptoms for which medical attention was sought for coughing, irritation, difficulty breathing, dizziness and disorientation. Thirteen individuals required medical treatment including overnight hospitalization.

Description: The product is marketed under the brand name of Tile Perfect™ Stand’n Seal “Spray-On” Grout Sealer. The date/lot codes are printed on the bottom of the can. All cans with date/lot codes starting with the following six digits are included in the recall: A20985; A30985; A10995; A20995; A30995; A11015; A21015; A31015; A11025; A21445; A31445; A11455; A21455; A31455; A11465; A21465. Units with other date/lot codes have been reformulated and are not subject to this recall.

Sold at: Exclusively at Home Depot Stores from April 2005 through June 2005 for about $10.

Manufactured in: United States

Remedy:: Consumers should not use the aerosol product and return it to the Home Depot for a full refund.

The Rocky Mountain Poison and Drug Center, in Denver, Colo., informed Roanoke Companies Group that two people had been hospitalized after using Stand’n Seal Grout Sealer, which led to this recall.

September 15, 2004, Release # 04-215: NCR Corp. Announce Recall of NCR Universal Inkjet Refill Kits -2004
WASHINGTON, D.C. - The U.S. Consumer Product Safety Commission announces the following recall in voluntary cooperation with the firm below. Consumers should stop using recalled products immediately unless otherwise instructed.

Name of product: NCR Universal Inkjet Refill Kits

Units: About 78,000

Manufacturer: NCR Corp., of Dayton, Ohio

Hazard: The product does not have required child-resistant packaging or required warning labels. The product contains ethylene glycol, posing an ingestion hazard to young children.

Incidents/Injuries: None reported.

Description: The recalled kits were sold in a clear plastic package with color or black ink. The black ink kits have model numbers 943264 and 999292 and the color ink kits have model number 999289 printed on the side panel above the UPC code. The two black inkjet sets contain four plastic bottles of black ink, two plastic bottles of cleaning fluid and one toolkit. The color inkjet set contains six plastic bottles of colored ink, two plastic bottles of cleaning solution and one toolkit.

Sold at: Big Lots and Walgreens nationwide from April 2004 through June 2004 for about $12.

Manufactured in: China

Remedy: Consumers should immediately remove the kits out of the reach of young children and return to the store where purchased to receive a refund or a free replacement kit.

January 20, 2004, Release # 04-066: Sassi America Inc. Announce Recall of Acrylic Primer for Nail Care-2004

WASHINGTON, D.C. - In cooperation with the U.S. Consumer Product Safety Commission (CPSC), Sassi America Inc., of Elk Grove Village, Ill., is voluntarily recalling about 72,000 bottles of acrylic primer used to prepare fingernails before applying artificial nails. This methacrylic acid primer is not packaged in child-resistant packaging, as required by the Poison Prevention Packaging Act.

Sassi America has received a report of a 2-year-old girl in Pompano Beach, Fla., who sustained severe external and internal chemical burns after swallowing the liquid primer.

The primer is packaged in amber-colored bottles with black caps. The model number 508, 509, 510, 511 or 512 is written on the front of the label. "SASSI" also is written on the labels. The primer was sold in .25-ounce, .50-ounce, 1-ounce, 2-ounce and 4-ounce size bottles. The acrylic primers were sold separately and in kits.

Grocery and beauty supply stores nationwide sold the primer from June 2000 through January 2004 for between $2 and $10.

October 15, 2003, Release # 04-010: John Wiley & Sons Inc. Announce Recall of "Candle and Soap Making For Dummies" Instruction Book-2003
WASHINGTON, D.C. - The U.S. Consumer Product Safety Commission announces the following recall in voluntary cooperation with the firm below. Consumers should stop using recalled products immediately unless otherwise instructed.

Name of product: "Candle and Soap Making For Dummies" Book

Units: 5,400

Manufacturer: John Wiley & Sons Inc., of Hoboken, N.J.

Hazard: The instructions in the book for making lye combine sodium hydroxide and water in an incorrect order. This could cause the mixture to bubble over, posing a burn hazard to consumers.

Incidents/Injuries: None reported.

Description: The paperback book is titled "Candle & Soap Making For Dummies." The book's cover is black and yellow and has a photograph of candlesticks and slices of soap.

Sold at: Bookstores and discount department stores nationwide from August 2003 through September 2003 for about $20.

Manufactured in: United States.

Remedy: Consumers should return these books to the store where purchased for a full refund.

July 17, 2003, Release # 03-161: DesignPac Announce Recall of Lamp Oil-2003
WASHINGTON, D.C. - The U.S. Consumer Product Safety Commission announces the following recall in voluntary cooperation with the firm below. Consumers should stop using recalled products immediately unless otherwise instructed.

Name of product: Oil Lamp Boxed Sets

Units: 4,000

Manufacturer: DesignPac, of Northlake, Ill.

Hazard: The bottles of paraffin oil in these sets do not have the child-resistant closures required by federal law.

Incidents/Injuries: None reported.

Description: The boxed sets contain a clear vase with pink or purple floral designs, and a 16-ounce bottle of paraffin oil. The packaging is labeled "Handpainted Oil Lamp Set" and "Floral Garden." Item number 054 11 0229 is printed on the packaging.

Sold at: Target stores nationwide from February 2003 through March 2003 for about $10.

Remedy: Return the oil lamp sets to any Target store for a refund.

April 19, 2002, Release # 02-144: SC Johnson Announce Recall of Glade® Extra Outlet Scented Oil Air Fresheners-2002
WASHINGTON, D.C. - In cooperation with the U.S. Consumer Product Safety Commission (CPSC), SC Johnson, of Racine, Wisc., is voluntarily recalling about 2.5 million Glade® Extra Outlet Scented Oil Air Fresheners. These Extra Outlet PlugIns® may have been misassembled during manufacture, which could pose a risk of fire.

SC Johnson has received five reports of misassembly and no reports of injury or property damage.

The Glade® Extra Outlet Scented Oil electric air fresheners come in two fragrances, Sky Breeze™ and Mystical Garden™. The air fresheners have a unique rotating plug-thru outlet, which allows other electrical devices to be used in the same outlet as the plug-in. The rotating outlet says "15 Amps Max" on the front and has a yellow disk on the back. The Extra Outlet Scented Oil units have a model number "SCJ079" on the back. No other products, sold under the Glade® PlugIns® brand names, are part of the recall.

Grocery and retail stores nationwide sold the air fresheners between January 2002 and April 2002 for between $4 and $5.

January 24, 2002, Release # 02-089: Value City, Schottenstein Stores Announce Recall of Gel Candle Kits-2002
WASHINGTON, D.C. - In cooperation with the U.S. Consumer Product Safety Commission (CPSC), Value City and Schottenstein stores, of Columbus, Ohio, are voluntarily recalling about 2,300 children's gel candle kits. When burned, the gel candle can melt the plastic candleholders included with the kit, posing a fire hazard to consumers.

CPSC has received two reports of consumers claiming the plastic candleholder melted and started a fire. One report involved substantial damage to the consumer's home, and a consumer reportedly suffered burns to his hands and feet. The other report involved minor damage to furniture.

The Gel Candles kit includes four bags of gel chips in bright colors, wicks, glitter, two glass holders, and two plastic candleholders. The candleholders are in the shapes of a bumblebee and a dragonfly, or a tulip and a daisy. The gel is scented in fruit flavors. The kits have model numbers "3041" or "3042" written on the box. Other writing on the box includes, "Express Ways!™ Gel Candles." "Scent- sational Colored Gel!" and "AGES 8 AND OLDER."

Value City and Schottenstein discount stores nationwide sold these gel candle kits from July 2001 through December 2001 for about $4.

Consumers should stop using these candles immediately and return them to the store where purchased for a refund. September 20, 2001, Release # 01-234: Blue Coral Announce Recall of Rain-X® Glass Cleaner and Washer Fluid-2001
WASHINGTON, D.C.- In cooperation with the U.S. Consumer Product Safety Commission (CPSC), Blue Coral, of Cleveland, Ohio is voluntarily recalling about 1.1 million containers of Rain-X® glass cleaner and washer fluid. The products, which contain methyl alcohol, are not sealed with child-resistant closures as required by federal law. Methyl alcohol, also known as methanol, is toxic and can cause blindness or death if ingested.

Blue Coral has received three reports of ingestion, including two children whose parents called the Blue Coral help line and were directed to immediate medical treatment.

The recalled products include three brands: Rain-X® Super Glass Cleaner Concentrate, which was sold in 10-oz. and 16.9-oz. plastic bottles, is a blue concentrated liquid that should be added to water; Rain-X® Plus Washer Fluid Concentrate, which was sold in 10-oz. and 16.9-oz. plastic bottles and single-use 1.2-oz. pouches, is a pink concentrated liquid that also should be added to water; and Rain-X® Washer Fluid Additive, which was sold in single-use 2-oz. pouches. The Glass Cleaner and Washer Fluid bottles have a measuring cup built into the cap. A label on the back of the products reads in part, "Blue Coral Slick 50 Ltd." and "Made in USA."

Hardware, automotive and department stores nationwide sold the recalled Rain-X® products from September 1998 through August 2001 for between $1 and $5.

Consumers should keep these recalled products out of the reach of young children. Consumers should call Blue Coral toll-free at (800) 416-1600 between 9 a.m. and 5 p.m. ET Monday through Friday for information on receiving replacement child-resistant caps and a coupon toward a Rain-X® product. July 16, 2001, Release # 01-199: Kole Imports Announce Recall of Contact Adhesive-2001
WASHINGTON, D.C.- In cooperation with the U.S. Consumer Product Safety Commission (CPSC), Kole Imports, of Carson, Calif., is voluntarily recalling about 90,000 packages of contact adhesive. The adhesive contains toluene, which can present a fire and poisoning hazard to consumers. The packaging of this product does not have the required cautionary labeling, as required under the Federal Hazardous Substances Act.

CPSC and Kole Imports have not received any reports of incidents. This recall is being conducted to prevent the possibility of injuries.

The recalled adhesive comes in a soft metal squeeze tube about 1" x 5" with a screw top. The tube is marked, "Super Contact Adhesive," and comes in yellow, orange, blue, and white. The UPC code is 7-31015-04471- 9.

Discount and "Dollar" stores nationwide sold the adhesive from January 2000 to January 2001 for about $1.

Consumers should stop using the adhesive immediately and call Kole Imports toll-free at (866) 251-0982 anytime to receive a full refund. Consumers also can write to Kole Imports at 24600 S. Main Street, Carson, CA 90745 for the refund. Consumers who write to the company should not mail the product to the company or the retailer. CPSC learned of this problem when the Texas State Health Department reported it to the Commission.

September 20, 2001, Release # 01-233: National Service Industries Inc. Announce Recall of Drain Cleaners-2001
WASHINGTON, D.C.- In cooperation with the U.S. Consumer Product Safety Commission (CPSC), National Service Industries Inc. (NSI), of Atlanta, Ga., is voluntarily recalling about 1.1 million bottles of drain cleaner sold in 64 oz., child-resistant bottles. The bottles can leak, allowing the contents of the cleaner to come into contact with consumers. These cleaners can cause irritation and burns to the skin and eyes.

NSI has received 16 reports of the bottles leaking, including three incidents involving skin irritations to the leg and hand, and 13 incidents involving property damage.

The following chart provides information concerning the four types of drain cleaners involved in this recall. Only 64-oz. bottles are affected.

The Drain Opener Clog Remover bottles and caps are white and the Hair Clog Remover bottles and caps are red. Each bottle has the brand name - "Zep Commercial" or "Enforcer" - on a label that also read in part, "Keep all chemicals out of reach of children."

Home Depot, Wal-Mart, Kmart and hardware stores nationwide sold these cleaners from January 2001 through August 2001 for between $3 and $6.

Consumers should stop using the cleaners immediately and contact the appropriate company to receive a free replacement cap.

Consumers should not return the recalled cleaner to the store where purchased.

July 2, 2001, Release # 01-183: The Flood Co. Announce Recall of Wood Strippers and Cleaners-2001
WASHINGTON, D.C. - In cooperation with the U.S. Consumer Product Safety Commission (CPSC), The Flood Co., of Hudson, Ohio, is voluntarily recalling about 18,000 bottles of Flood StainStrip and FloodPro Stripper/Cleaner sold in gallon bottles. Some of these bottles can leak at the cap, allowing the contents to come into contact with consumers. These strippers and cleaners can cause burns to the skin and eyes.

The Flood Co. has received two reports of bottles leaking that were shipped to retailers, but no injuries have been reported.

Only gallon bottles of Flood StainStrip and Stripper/Cleaner are part of this recall. Both bottles show the Flood logo in the shape of a paint brush.Writing on the StrainStrip Exterior Stain Stripper label reads, " REMOVES ALL FINISHES," and "DANGER! CAUSES EYE, SKIN AND MUCOUS MEMBRANE BURNS." The Stripper/Cleaner label reads, "4 Products In One!"

Paint and hardware stores nationwide sold these Flood products from March 2001 through June 2001 for between $15 and $20.

Consumers should stop using the recalled cleaners immediately since the bottles can leak, and call the Flood Co. hotline. The hotline will help consumers determine if their bottle is leaking, and provide instructions on how to dispose of leaking bottles and get a refund. January 11, 2000, Release # 00-052: Reckitt & Colman Inc. Announce Recall of EASY-OFF Oven Cleaner-2000
WASHINGTON, D.C. - In cooperation with the U.S. Consumer Product Safety Commission (CPSC), Reckitt & Colman Inc., of Wayne, N.J., is voluntarily recalling about 50,000 cans of EASY-OFF Heavy Duty Oven Cleaner. The oven cleaner's contents can spew out because of an improperly attached valve assembly that can separate from the can. Direct contact with the contents poses a risk of chemical burns to consumers' skin and eyes.

Reckitt & Colman has received 18 reports of these cans spewing oven cleaner, resulting in 12 reports of chemical burns to consumers' skin and eyes.

The 16-oz. aerosol cans of oven cleaner being recalled are yellow and have a solid yellow plastic cap. "EASY-OFF" is in bold black letters at the top part of the can, "HEAVY DUTY" is written in white letters inside a red block, and "Original" is written in white letters over a blue background. Only the oven cleaner with lot number B9305-NJ2 printed on the bottom of the can is part of the recall.

Supermarkets, mass merchandisers, hardware, drug and other stores nationwide sold the oven cleaner from November 1999 through January 2000 for about $3 to $4.

Consumers should not remove the cap of this oven cleaner. Consumers should wrap the recalled can of oven cleaner in several layers of newspaper, take it outside and dispose of it with the regular household trash. The company will send consumers a replacement oven cleaner coupon. January 18, 2000, Release # 00-054: The Sherwin-Williams Co. and Brockway Standard Inc. Announce Recall of Furniture Refinisher-2000
WASHINGTON, D.C. - In cooperation with the U.S. Consumer Product Safety Commission (CPSC), The Sherwin-Williams Co., of Cleveland, Ohio and Brockway Standard Inc. of Atlanta, Ga., are voluntarily recalling about 3,000 cans of Formby's Conditioning Furniture Refinisher. The product and its vapors can seep or leak from the nozzle base at the top of the can, posing a risk of fire and chemical injuries.

The firms have received one report of an employee suffering minor burns when refinisher vapors ignited in a retailer's distribution center.

Formby's Conditioning Furniture Refinisher, used to refinish wood furniture, is sold individually and as part of Formby's "Introduction to Refinishing Kit." The 32-ounce metal can reads, "Formby's Furniture Workshop" and "Furniture Refinisher" in white lettering on a burgundy and mostly green background. A depiction of Homer Formby appears on the face of the can. The product can be identified by a code on the bottom of the can written in black ink with one of the following lot number sequences: A961600, A961610, A961620, A961630, A967270 and A969330. Cans with blue ink codes are not subject to this recall.

Discount department, hardware and hobby stores nationwide sold the refinisher from October 1999 through January 2000. The individual cans sold for about $13 and the kits sold for about $16.

Consumers should stop using this refinishing product immediately, and place the product in a well-ventilated area. Since it can leak, care should be taken to keep the product from spilling by always keeping the container upright. Also, do not store near any source of ignition, such as near a gas-fueled appliance. Consumers should call (800) 523-9299 between 9 a.m. and 4 p.m. ET Monday through Friday for instructions on how to handle and return the refinisher for replacement or a refund.

August 17, 2000, Release # 00-165: The Penray Companies Inc. Announce Recall of Starting Fluid-2000

WASHINGTON, D.C. - In cooperation with the U.S. Consumer Product Safety Commission (CPSC), The Penray Companies Inc., of Wheeling, Ill., is recalling about 113,000 cans of starting fluid, which is a lubricant to aid starting gas and diesel engines. The cans containing the starting fluid can leak as a result of internal to external corrosion. The starting fluid is extremely flammable and the leaking fluid poses a fire hazard. Direct contact with the product also can irritate the consumer's eyes and skin.

Penray has received three reports of cans of the starting fluid leaking. No injuries have been reported.

The recalled starting fluid was sold under the Penray, Super-X and Service Pro brand names in 10.1 ounce cans. The recalled cans have a number printed on the dome of the can ranging from 9305 through 9365. For consumers who purchased the starting fluid by the case, the production date is written on the side of the case. The cans are mostly black with white and red writing and black plastic caps.

Automobile repair and rebuild shops sold the starting fluid nationwide from November 1999 through August 2000 for $2 to $3.

Consumers should stop using the recalled starting fluid immediately. Place it in a well-ventilated area away from sparks or flames. Consumers should call Penray toll-free at (800) 323-6329 between 8:30 a.m. and 4:30 p.m. CT Monday through Friday for information on how to dispose of these cans, and how to get a refund.
